This is a collection of life and death certificate applications from around the world. With each, I've removed all typography to highlight the formal decisions made by the forms' designer. 

The forms have been printed on stock office supply paper and bound in a custom binder that accommodates the varying paper sizes (letter, legal and A4) from around the world. The birth certificate is noted with an upper tab and and the death certificate with a lower. My fascination with the forms grew from a curiosity about the cultural practice of needing to fill out forms to enter and exit the social world.
